Incident: Some users were experiencing an 'Invalid Token' error when attempting to make requests with a valid API token following a system update.

Root Cause: There was a failure to identify existing user profiles linked to MediRecords API tokens after the update. When the user failed to be identified, the token was removed. 

Immediate action to be taken: MediRecords will strengthen our testing procedures prior to release. In the event that testing fails, production release will not continue.
